Accidents happen and it sounds nasty. What weight did the guy have smashing into his face?

I was spotting at the British Nationals a few weeks back and me and one other guy caught quite a few failed attempts, one of which was 290.5kg without incident. If the spotters are treating each lift like somebody's life depends on it (which it might do) then these accidents are very, very rare. Did the shirt blowout or what?
