- Fingers kept locked under the bar; spotters "follow" the weight both down and up, never allowing more than ~4" between the bar and their hands.

In my opinion that is the most important thing. The spotters have to stay right with the bar with their hands under the bar.

Things happen so fast with a miss these days that you can't move in quick enough to catch the bar, you have to already be there.

The other thing is that in the "old days" almost everyone would make their opener - 2nds were about 60 percent made and thirds was when you had to really expect to see some misses. These days people miss their openers so often you have to really be on the ball.
