You're right, that is kind of silly or at least its an over simplfication. Guys in Rages get out of the groove - I've done it myself - I also ripped 2 in a row at Masters last year. That was me trying to get into a way too small shirt and pulling it way down. As I pulled down, the shirts ripped and the bar came down. The difference I think is that for me the bar came straight down to the chest.
Most of the guys that I've seen have accidents in Furys have had the bar flip back towards the head/neck sort of like a tricep extension gone bad. It's possible to do that in a Rage - I did someting like that in training once trying to figure out a shirt but the ones I've seen in meets have been Furys.

I think the bigger issue is that the spotters need to stay right with the bar with the side guys having hands clasped right under the sleeves the whole time. IF you've got that 3 guys is fine even for a 700 lbs attempt. Maybe a 900 needs 5 guys but we don't have a lot of those in single ply drug tested meets.
