Jimmy... I do not have the DVD nor was I at the event that you are commenting on. In my opinion if the head referee is consistently calling depth while the sides are giving whites then it is up to the jury to instruct/remove that referee.

Uneven extension or up/down? Different calls... your description sounds like an up/down red light (based on your assumption that it was caused by a slight [and allowed] uneven extension).

As a side referee I do not use the end of the bar as my reference point for either up/down nor uneven extension. I would find it hard to do so (with spotters in the way, have to watch the motionless on the chest, etc). I prefer to watch for those infractions at the point where the hands contact the bar. Using those reference points your description does not come into play.

From my experience side referees have a difficult view point to evaluate uneven extension.
