It seems that the Willet case began the enforcement of the rule. There was a huge stink made by members of this organization about Willet being removed from the team. Mostly because she was not given prior warning.

Since then it seems the IPF is first giving a warning before suspending. Not sure if this is true in the case of Mr.White, but similar warnings have been given recently.

Willet seemingly set the standard so that there needs to be a warning before further action.
