I have seen it happen when the lifter was made to hold the bar until she dropped it. I personally have gotten a down signal on the deadlift 3 times when I wasn't finished with the lift, hadn't gotten one shoulder back. I knew it and the 2 side referees knew it, the head referee thought I was locked out each time. Guess what, I got red-lighted and on one of them, it was an American Record and a PR. I could have protested but sometimes, you just get bad calls. But you have to think of the times when you didn't have one knee locked but the refs didn't see it. It all works out in the end.
