"I havent seen many bad bench injuries. Thumbless or not. I have seen bars drop on bellies but each time they walked away."

I have. I've seen a guy drop the bar and break ribs more than once. Didn't really walk away. Kind of rolled off the bench. I've seen a guy dump the bar right on his foreheard and put a hole in his dome that I could actually LOOK INTO (and look I did.)

"I have seen a bad squat injury. Is it also because of equipment? that scares me more than anything."

No. LOL I've seen suits blow and guys were fine. Took their next attempts. I've seen injuries in raw meets, single ply meets and multi ply meets. When it's your time, its your time.

There is nothing to be scared of. I've fallen forwards, backwards and even had the bar roll over my head. The key is to a) stay with the bar and b) get away from the bar as soon as possible.

I know those seem mutually exclusive but they aren't. If the bar rolls over your head, just do a static good morning and do not let the spotters put it on the floor. Insist that it goes to the bench hooks. That way you just missed but you didn't dump the bar.

If you fall forwards, at some point the bar will hit the racks and you will be catapulted to the safety of the head refs' feet or lap. Your arms will feel like they are ripped off but you will be ok..... eventually.

If you fall backwards, stay with the bar and hope the spotters are awake. (note: do most of your falling at Nationals - they have great spotters.)

"People keep trying to limit the bench shirt and want to make new rules to hold back progress but nobody is talking about squat suits. "

The squat is God's lift. It is to be loved and admired. The bench is just what you do before the deads start. Personally, I think it should be eliminated altogether.
