Bench Press - Spotters

Just a quick question... CPU (Canadian IPF affiliate) nationals was 2 weeks ago. A well run and enjoyed affair but it was marred by one accident in the bench press.

One of our more seasoned lifters was attempting a new national record and came out of groove. With the new shirts if you badly miss the shirt is coming at your head/throat.

In a split second before the spotters could react the bar fell 3-4" and injured the lifter (who went to the hospital). It could have been worse but resulted in mostly some broken teeth.

The IPF rules allow only 3 spotters on the platform at a time for bench press. Is this reasonable in this age of new bench shirts? And where should spotters be located for safety? With the head spotter backing out of the way this usually leaves the 2 side spotters as the primary safety.
